Intake Interview and Intake Form

The Intake Interview Form is an opportunity for students to review their documentation with the Accessibility Officer, as well as to discuss any needs and concerns that need to be addressed. Students can inform the Accessibility Officer of any updates that have been made to their documentation, and review their academic history to identify any potential aids that may be available to them. With the student’s written consent, Accessibility Services will contact the students’ instructors, and may also seek further information from or consult with professionals or experts, either internal or external, to the University.

Intake Interviews could cover include topics such as:

  • Campus physical accessibility
  • Grant opportunities for students with disabilities through StudentAidNL
  • Classroom accommodations
  • Accommodations for transitions to Marine Institute
  • Accommodation for work placements
  • On-campus housing Accommodations
  • Availability of assistive technology and alternative format materials
  • Accommodations for tests/exams and the process for booking accommodations for tests
  • Emergency evacuation guidelines
  • Accommodations for orientation
  • Any other on-campus and community resources or services
